Assessments

Healthy Aging Solutions will provide an unbiased, complete geriatric nursing assessment to identify the client’s actual and potential problems, patterns, strengths and needs related to wellness and/or illness.

Assessments can include:

All assessments are conducted where the client lives or is staying temporarily. Based on the assessment, desired outcomes are specified and a therapeutic care plan is generated.

Home Care

Healthy Aging Solutions researches appropriate options in home care to inform you of the quality, cost, and availability of your care choices. A summary of both live-in and hourly care, through private and agency sources, is provided. Screening is done and arrangements are made for home care needs, such as personal care, meals, medical equipment, and other in-home support services. Home or facility visits can be done to monitor the care being given by caregivers.

Placement

Healthy Aging Solutions will make a recommendation about the level of care that should be provided for the client. A full site evaluation of prospective care centers is provided, including information regarding quality, cost, and availability of care centers. A report on available governmental surveys and violations of federal and state standards is also provided. When a new accommodation has been chosen, Healthy Aging Solutions can assist with arrangements for the move and monitor adjustment of the client to the new surroundings. After placement, the Nurse Care Manager can remain an advocate for the senior's long-term or short-term care.

Case Management

Healthy Aging Solutions will develop a personalized, therapeutic Care Management Plan that is revised on an ongoing basis. Through coordination of care and assistance with access to timely and appropriate health care services, continuity of care is ensured. The Nurse Care Manager acts as a liaison and works collaboratively with various health professionals to help others understand the client’s health care situation and needs. The needs of clients and families are dynamic and may vary. Thus, ongoing evaluation of choices is needed to promote self-determination.

Health Management

In caring for older adults, nursing care requires a special insight based on a solid understanding of the physiology of aging and such complex issues as geriatric syndromes like falls and dementia. At the same time, the Nurse Care Manager can assist older adults in maximizing health by promoting healthy lifestyle habits and addressing preventive health care. The Care Manager can assess health changes and needs at intervals and address concerns.

Medication management includes a medication review and assessment for possible factors influencing the client’s ability to adhere to a medication regimen. Client teaching is guided by what the client needs to do and know in order to take medications safely. The Care Manager also communicates with the client’s primary care provider and specialists regarding medications.

Health Education is an important part of managing health care needs. An interactive, collaborative process between the Nurse Care Manager and the client facilitates progress toward the client’s goal of assuming responsibility for his or her health and self-care to whatever extent possible.

Fees

Fees for services will be discussed during the first contact with the Nurse Care Manager. The time required to complete a comprehensive evaluation of the client and his or her needs varies depending on the complexity of the situation. Pricing is applied accordingly. A few hours may be required to research options and discuss these with the client and family, helping them to take the necessary steps to implement the care plan. Most people require geriatric care management only intermittently once support services are in place. Following the initial assessment, the Nurse Care Manager will help you carefully estimate the ongoing cost of service delivery, if needed.

Services are not covered by Medicare or Medicaid. Most health insurance policies will not cover geriatric and chronic illness management services. Long Term Care insurance may reimburse for some care management services.
